Care Plans

Our Doctors provide care plans for Mental health and Enhanced primary care based on the individual patient’s eligibility.

If your appointment is for a care plan, please advise the receptionist when making an appointment and we will provide you with a longer appointment. Appointments for care plans are only available Monday to Friday.
